People seeking a structured ADHD assessment via telehealth with a doctor-led approach.
Do I need a referral?
If you have a referral letter, past reports, or previous diagnoses, bring them—they can be helpful context. If you're unsure what you need, email support@adhdgpclinic.com.au.
How long does the intake take?
Most people complete the intake in 10-25 minutes. You can pause and return later using the same link; progress saves as you go.
How long is the appointment?
Appointment length varies and is shown at the time of booking. Telehealth times are chosen to allow a thorough assessment.
Can I do this from home?
Yes. Find a quiet, private space with a stable internet connection. Headphones can help with privacy.
Can someone help me fill it out?
Responses should reflect your own experience. You can ask for help with reading or typing if needed. In some cases an observer questionnaire may also be provided.
Will I get a written summary or letter?
If clinically appropriate, your doctor may provide documentation such as a summary letter. The type of documentation depends on your situation and findings.
Do you prescribe medication?
Medication may be discussed where clinically appropriate and in line with relevant regulations. Sometimes the best next step involves coordination with your usual GP or another clinician.
What if my questionnaire link does not work?
If your link is invalid or expired, contact support at support@adhdgpclinic.com.au to request a new questionnaire link.
